Your lab tests can be performed at: 

• Any laboratory of your choice (insurance may not cover)
• A laboratory within your insurance network (Insurance will usually cover some or all but not always)

Possible coverage requirements and network status: 

• Tests must be performed at an in-network laboratory only 
• Tests must be performed at an in-network laboratory AND must be ordered by an in-network healthcare provider

• If they cover labs ordered by out-of-network providers 
• What your expected out-of-pocket costs will be
• Which laboratories are in your network
